Aim: In this study, we review the patient demographics, imaging appearances, and pathological entities which arise in the ischium with the aim of facilitating a differential diagnosis. This allows appropriate management to be instigated in a timely fashion, enabling lesion to be stratified into those that can be managed locally or referred to a specialist center. Materials and Methods: A retrospective search of our oncology and radiology database was performed to identify primary ischial lesions. Results: The search revealed 82 cases with primary involvement of ischium. The most common benign tumor was aneurysmal bone cyst in patients under 40 years and osteochondroma in patients over the age of 40 years. Metastasis was the most common malignant tumor in both cohorts (<40 years and over 40 years).
